Rich piana was a well known bodybuilder who took extreme cycles of steroids going from Testosterone to Trenbolone and many other things such as abusing insulin ( you can find his stack on archived instagram posts)

Piana collapsed and passed away from heart issues in Florida 2017 at age 46 after being admitted to a hospital the autopsy was preformed

Notable thing is he had a enlarged heart weighing in at 670g when a normal heart weights around 300g max and there were many things wrong with his lungs and arteries

The photo included of him is the last photo taken/posted of him alive

Hemopneumothorax on the right. Shrapnel wound of the pericardium. Perforating wound of the right atrium appendage. Hemopericardium. Cardiac tamponade. Multiple fragmentation blind non-penetrating wounds of the abdomen and lumbar region. Multiple fragmentation blind wounds of the lower extremities with a fracture of both bones of the right and left leg.

Done:

Anterolateral thoracotomy on the right.

Opening the pericardium and suturing the wound of the right appendage. Suturing wounds of the right lung. Sanitation and drainage of the right pleural cavity. Dressing wounds.
